the Hoag Annual Wellness Assessment
designed for the Costa Mesa Police Department

The Costa Mesa Police Department has partnered with Hoag to provide a thorough nutritional, fitness and lifestyle assessment customized for law enforcement personnel.

Annual Wellness Assessment

Clinical Components:

  • Medical and Lifestyle Questionnaire
  • First Responder Specific Complete Blood Panel (Including: Lipid Panel, Blood Sugars, Inflammatory Markers, TSH, PSA (45+), CBC, CMP)
  • Physician Blood Panel Review
  • Follow-up report with physician notes, recommendations and referrals

Fitness Components:

  • Fitness and Nutrition Consultation
  • Digital Movement Screening
  • In-Body Body Composition Analysis
  • Hand Grip Strength Test
  • Upper-Body Strength & Endurance Test (Push-Ups)
  • Core Strength & Endurance Test (Plank)
  • Gas-Exchange Vo2 Max Treadmill or Cycle Test (Sworn Personnel only)
    *Treadmill Stress EKG if Risk Stratified or Doctor recommended only
  • Follow-Up appointment with Wellness Report (All test results, exercise prescriptions, dietary recommendations, doctors note and other recommendations)

Exams Reminders: Wear comfortable shoes and fitness/workout attire

  • Once you schedule, you will receive two emails from Hoag regarding your exam.

    • Email #1: This will come from Hoag adobesign@adobesign.com and instruct you to complete an electronic health questionnaire. Please complete and submit online. 
    • Email #2: This will come from our Program Coordinator (within 48 hours) and will include your lab order and instructions. Please complete your blood work at least 5 days prior to your exam at a LabCorp location convenient for you. You can schedule an appointment online at LabCorp Locations or walk-in at your convenience.
