Accomplishments

Hoag saw a 10 percent reduction in overall readmissions; a 16 percent reduction in pneumonia readmissions; an 18 percent reduction in sepsis readmissions; a 10 percent reduction in Inpatient Mortality; sustained improvement in 30-day mortality; a 20% reduction in HF 30-day mortality; a 30% reduction in COPD readmissions; a 10% improvement in “safe use of opioids;” and a 92% improvement in spontaneous awake trials completing implementation of the full ICU liberation bundle. This is all thanks to clinician-driven quality (QI) improvement, the implementation of QI methodologies, and the alignment of clinical quality goals between front line physicians and staff.

Hoag had zero substantiated never events/never events with care violations and zero never events with care deficiencies.

Hoag achieved its stretch goal of reducing medication safety errors by 65 percent in 2024 over 2023.

Hoag achieved its target goal of reducing falls with significant injury by 20 percent in 2024 over 2023.

Hoag achieved its stretch goal to reduce both Hospital Acquired Infections and Surgical Site Infections in 2024 over the previous year.

Hoag launched its Serious Safety Event Committee, which will help identify and trend a broader scope of safety events.

More than 300 participants from multiple disciplines attended repurposed and revitalized Grand Rounds with several CME accredited events that focused on clinical quality improvement featuring best practices from national leaders.

Hoag launched the Hoag Clinical Quality Advisory Commission to focus on primary and subspecialty ambulatory care quality and raise the bar for employed physicians.

The biannual Skilled Nursing Home and Home Health Agency Hoag collaborative event brought together 14 agencies to discuss readmissions and improve transitions of care, resulting in a 15 percent reduction in SNF/HHA readmissions.
