Please join us for a discussion on Hoag Palliative Care and Hoag Hospice resources, led by Executive Director of Hoag at Home Jeff Hammond. He will provide an overview of palliative care versus hospice appropriateness. 

Additionally, Hoag Clinic’s Quality Improvement team will review HEDIS measure guidelines, with a focus on cardiovascular-related measures, such as blood pressure control, medication adherence and statin therapy for patients with cardiovascular disease. We will also share best practices for improving measure performance and review proper documentation necessary to close quality care gaps.
