Hoag HIE
Here’s how it works:
When your Primary Care Provider refers you to another physician, you will no longer need to carry paper copies of your health records with you. Instead, the information will be sent securely to the new physician through the HIE. Not only does this help your physician to provide better care for you, it saves you time and reduces your out-of-pocket costs.
Hoag makes every effort to protect your health information. Only those physicians and providers who provide you with health care services will be allowed to view your test results and medical reports. The HIE complies with Hoag’s Notice of Privacy Practices. All participants in the HIE are obligated to protect your information, by adhering to the requirements of the Health Insurance Portability and Accountability Act of 1996, as amended (“HIPAA”); and other applicable law.
Here are some reasons to participate in the HIE:
- When your physician sends you for a test, the HIE delivers your results to your physician immediately.
- Your physician receives test results as soon as they are generated. If the results indicate an abnormality, your physician will know immediately and can begin treatment sooner, if applicable.
- If you have an accident and end up in the emergency room unable to speak, the physicians and nurses may access lifesaving information about you through the HIE.
You have the right to opt-out of the HIE at any time. If you choose to opt-out, your health information will not accessible through the HIE. To opt-out or for more information, contact:
